Pursuant to Article 13 (1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation), OJ.EU.L.2016.119.1 (hereinafter: GDPR), we kindly inform you that:

1. the administrator of the personal data collected is 5R INVEST Spółka z o.o. with its registered office in Wrocław, ul. Kanonia 9/13, 50-328 Wrocław;
2. each time personal data is collected, you will be informed of the purpose, legal basis and duration of the processing;
3. where personal data will be processed on the basis of your consent to the processing of personal data, you have the right to withdraw your consent at any time without affecting the lawfulness of the processing carried out on the basis of your consent prior to its withdrawal;
4. the recipients of personal data may be other entities to whom the controller entrusts the processing of personal data;
5. the controller has no intention to transfer personal data to a third country or to an international organisation;
6. you have the right to request from the controller access to your personal data, rectification, erasure or restriction of processing, as well as to request data portability, which will be implemented in accordance with the principles set out in Chapter III of the GDPR;
7. you also have the right to object to the processing, which will be exercisable under the terms of Article 21 of GDPR;
8. in the course of processing your personal data, no decisions concerning you will be made automatically and no profiles will be created, which means that the actions referred to in Article 22(1) and (4) of 
GDPR will not be taken;
9. if you find that the processing of your personal data violates the GDPR, you have the right to lodge a complaint with the supervisory authority, which in Poland is the President of the Office for Personal Data Protection; (registered address: ul. Stawki 2, 00-193 Warszawa);
10. the provision of personal data is voluntary, but necessary in order to fulfil the purpose stated in each case, and without providing such data it is not possible to fulfil this purpose.
